What is the Netherlands Passenger Locator Form

The Netherlands Passenger Locator Form is a short online health declaration that’s required of every incoming traveller to the Netherlands aged 12 and older. The form must be submitted online before the planned departure to receive the confirmation of its completion on time for boarding.

The Netherlands PLF is available to be completed online and comprises three concise sections to be filled out with the required data, including the traveller’s personal information, their health status, and flight and travel details.

Completing the online Passenger Locator Form is necessary during the ongoing pandemic for any traveller aged 12 or older planning a visit to or from the Netherlands. The online health declaration is part of many other safety measures and entry requirements established by the Dutch Government to keep the Covid-19 situation within the country under control.

Who needs to fill out the PLF for the Netherlands

The Netherlands Passenger Locator Form is required for any person aged 12 and over wishing to cross the Netherlands border either way.

Travellers crossing the border must have their completed forms prepared for the border or airport officials to check.

The completed form ensures the Dutch health authorities that your visit does not pose any threat to the general public and will not add to worsening the Covid-19 situation within the country.

When should you fill out the Netherlands PLF

The Netherlands PLF should be filled out within 72 hours to no later than 24 hours of the planned departure.

Completing the form no later than 24 hours of the intended departure time ensures that you will receive the confirmation of its completion on time.

How to fill out the Netherlands Passenger Locator Form

The Netherlands Passenger Locator Form can be easily completed online providing you have the following:

  • An active email address
  • Internet connection
  • A phone, laptop, PC, or any other device to complete the process
  • Details of your visit to the Netherlands, such as your flight number

 

The first part of the Netherlands PLF is answering three yes/no questions relating to your health status:

  1. Have you been diagnosed with Covid-19 or any lung infection/or is there a possibility that you may have one?
  2. Are you currently in quarantine because of having contact with someone who has been diagnosed with Covid and you are not fully vaccinated?
  3. Have you been having any of the following symptoms (coughing, symptoms of a cold, shortness of breath, a fever, loss of smell or taste)

 

Once this section of the form is completed, provide some basic information of your upcoming trip to the Netherlands, such as your flight number, and your personal details, such as your date of birth and name.

Please note that all the details must be entered into the Netherlands Passenger Locator Form correctly to be considered valid and accurate for the Dutch Government.

After applying

After applying, each traveller receives a PDF confirmation of completing the Dutch PLF via email.

You can then download the received confirmation and either print it out or keep it in an electronic form to present it upon boarding the aircraft or on arrival.
